Individual Supports

UCP is a qualified vendor of the Real Life Choices program. Via Real Life Choices, people with disabilities who are enrolled in the program through DDD enjoy a self-directed approach that gives them and their families the budget and the freedom to customize supports and services. Service currently available for purchase through Real Life Choices include Hotel Respite, Recreation, Community Mentors and Individual Supports. During fiscal year 2010, over 7,492 hours of service were provided to 29 individuals through Real Life Choice funding.
